Determined that Ryujin Industries quest RI04 "Background Checks" fails to start if you have previously killed the elevator guard. Corrected quest alias conditions to allow for this actor to be dead. Quest now starts as expected after the quest "Top Secrets".

Interesting in rolling this into the community patch and joining for the long term so long as I become a contributor.   Determined that Ryujin Industries quest RI04 "Background Checks" fails to start if you have previously killed the elevator guard (15C9BD). Corrected quest alias conditions to allow for this actor to be dead. Quest now starts as expected after the quest "Top Secrets".

If you've already failed to start the Background Checks quest, some console commands will be needed to get you back on track:

startquest 1FB85C setstage 1FB85C 99

Go up to the executive office, the next command will trigger the expected scene

setstage 1FB85C 100

The above could be scripted into a quest to fix existing games, but wasnt sure if this was feature creep for what could be done via console. When console modding is out if this isnt fixed via Bethesda that may be needed.
